The Teamsters Local 808 Welfare Fund participates in the Empire BlueCross BlueShield network of service providers. If you need to find a doctor or specialist near you, these instructions will help walk you through the Empire BlueCross BlueShield website.

1. Click on the following link: Search for a doctor.
2. Select your plan type (this information is on your card)
3. Select the provider type and specialty if directed
4. Click Next
5. Enter your home or work zip code (we recommend you limit your search to 1 or 5 miles due to the large number of doctors in the NYC area)
6. Select other criteria, such as preferred language. Then click "Search."
